Individual Muster IN Roll 2/25/64 Baton Rouge la Entered the service as Private Co E 7th Ky 8/19/61. Promoted to be Sergt 1/62 of same Co. Promoted from sgt vice White promoted to be Capt by Commission from Gov of Ky dated 6/3/63. Discharged as Sgt by order from HdQtrs 13A.C. dated 6/22/1863. MUSTEROUT 10/5/64 at Louisville Ky Surgeon Certicificate John T Bates, First Lieut, Co E 7th Regmt Ky Vol Inf having applied for a certificate on which to ground the cause of absence. I do hereby certify that I have carefully examined the officer and find that he is laboring under gunshot wound of the right leg and chills and fever, in consequence thereof he is in my opinion unfit for duty. I further declare my belief that he will not be able to resume his duties in a less period than 20 days and that he is unable to travel at present.
