"I certify on Honor that Jesse Powers a private of Lt. Jacob P Phipp's Company A, of the 8th Regiment of Kentucky Volunteers of the state of Kentucky born in Whitley County state of Kentucky aged 45 years, 6 feet 9inches high; fair complexion, blue eyes, dark hair, and by occupation a farmer, having joined the company on it original organization at barbourville, and enrolled in it at the muster into the service of the United States at Camp Robinson on the first day of October 1861 for the term of three years unless sooner discharged and having served honestly and faithfully with his company in Kentucky to the present date, is now entitled to a discharge by reason of DEATH, he died in a rebel prison, Va, Nov 5 1863.
